POLICY CONTRACT

PLEASE READ AND AGREE TO THE POLICY CONTRACT

ENROLLMENT FEE: There is a $5 fee to enroll in summer private lessons and summer group lessons/classes.

SUMMER TUITION PAYMENT (Credit Card or Debit Only): Tuition submitted for the summer is processed through a one-time credit card OR a one-time pre-authorized auto bank draft. Your full summer tuition will be automatically processed on June 1st or upon registration thereafter. If your full tuition is more than $275, please contact the studio if you need a summer payment arrangement.

NON-SUFFICIENT FUNDS (NSF) FEE: In the case of an ACH Transaction being rejected for Non Sufficient Funds (NSF) I understand that Studios of Sarah Strout may, at its discretion, attempt to process the charge again within 5 days, and I agree to an additional $25 charge for each attempt returned NSF which will be initiated as a separate transaction from the authorized recurring payment.

REFUNDS: If a group class is canceled due to insufficient enrollment, a full refund, including your $50 deposit, will be given. No refunds are given for missed lessons or for group classes. If a group falls below 3 students the lesson time will be reduced accordingly, either 20 or 30 minutes depending on the number of students present. Credits cannot be carried into the academic year and are not transferable to other family members.

ARRIVAL/DEPARTURE: Students are expected to arrive on time so that class may begin as scheduled. Arriving late will shorten the class accordingly. Your teacher will meet you in the lobby at your scheduled lesson time. Teachers do not supervise students before and/or after their assigned lesson time.

STUDENT ABSENCE: Tuition reserves a student’s private lesson time each week. Private lesson students are entitled to two (2) make-up lessons during the summer session. In order to be eligible for a make-up lesson, lessons must be canceled by notifying the Studio before 8:00 AM. Notification can be sent via email or phone (info@stroutstudios.com/ 214-370-9485). Group classes are not eligible for make-up lessons due to the nature of the program. PLEASE NOTE: Last minute cancellations and no-shows will count as a lesson and are not eligible for make ups . We cannot make exceptions to this policy.

TEACHER SUBSTITUTIONS: Studios of Sarah Strout reserves the right to provide another qualified teacher if the regularly scheduled teacher is ill or otherwise unable to teach classes. If a teacher is out and the studio cannot arrange a substitute that missed classes will be made up.

PHOTO RELEASE: By enrolling with this form, I agree that the Studios of Sarah Stout may use photographs of my child for any lawful purpose, such as for publicity, advertising, and web content. If not, you must notify us in writing.
